Here is the application for background checks. Complete and submit this form, then you will receive an email from Ministry Safe with a form to complete and submit. All employees, coaches, volunteers, and chaperones are required to submit this application. Please contact the school office to see if your approved background check is up to date. You also need to complete this yearly online training. After going through the slide deck, you will need to click on the link at end of the presentation and complete the form to receive credit for completing the training.
